What We Do

The Department of Outreach Services promotes the growth and maintenance of HSCSN/NET membership and is responsible for planning and participating in programs to increase community awareness of HSCSN/NET benefits and services.

 

Outreach Services makes sure that SSI-eligible individuals are aware of benefits and services available to them through our program. Efforts are made to find existing members and provide assistance to help them improve their social and environmental needs. The Outreach Services Department has started several support groups to address the special needs and concerns of families and communities.

 

Outreach Services include:

 

* Visiting homes of new SSI recipients to tell them about our benefits and services and to conduct a health assessment survey for enrollment into our program.

 

* Contacting our members for appointments and record updates.

 

* Providing assistance to members in their efforts to improve environmental and social concerns.

 

* Increasing community awareness of HSCSN/NET benefits and services.

 

* Cooperating with community service and faith-based organizations to improve the health and well-being of special needs children and families.

 

* Coordinating support groups to provide parents and caregivers of children with special needs with mentoring and advocacy assistance.
